Survey on Transitions to Adulthood


A survey on transitions to adulthood titled ‘Talking about Tomorrow’ is now available to complete.  This survey is for parents and carers of young disabled people aged 14 and over, who are based in England. We want to hear about your current and past experiences at the time when your child or young person was transitioning to adulthood.


This survey is part of a project funded by the Department for Health and Social Care (DHSC) and delivered by Contact, Carers UK, National Development Team for Inclusion, and National Network of Parent Carer Forums.

Your responses will help us to develop resources that can be used by parents and practitioners as the young person they care for transitions to adult life. We want to improve the experience of the young person but also of the parent carer at this important time.
