NICE Quality Standard for Transition from Children's to Adult's Services

Following the publication of guidance on transition from children's to adults services, NICE have released related a quality standard. This quality standard covers all young people (aged up to 25) using children's health and social care services who are due to make the transition to adults' services.

The purpose of this booklet is for others to find out crucial information about a child or young person with high support needs in a short time-frame. It could be used by a new support worker at home, staff at a short-term break service, a learning assistant or teacher at school or a nurse if the person has had to be admitted into hospital. It also contains essential information should an emergency situation occur – whether it is a social care or medical emergency.

The Association for Young People's Health have released a toolkit for school nurses, designed to improve young people's health literacy.

Produced with support from Public Health England, the toolkit is designed to help school nurses improve the health literacy of children and young people using simple and cost effective methods.
