Pathfinder Support
We will be providing support to Pathfinders that are working on
the Preparing for Adulthood strand of work (transition). This
support will form part of the wider Pathfinder Support Programme.
Mott MacDonald have been appointed as the pathfinder
support team.
All pathfinders will be covering transition in their development
of a 0-25 single assessment and education, health and social care
plan and 10 pathfinder sites (15 local authorities) will be looking
at preparing for adulthood in depth with support from our
team.
We will develop a tailored programme of work with each pathfinder,
aimed at developing the 0-25 plan, increasing local provision,
working closely with families and young people, focusing on
outcomes and using resources effectively to ensure that young
people move into adulthood with jobs, independent living, good
health and social inclusion.
Menu of Support
We have put together a 'Menu of Support', designed to support
pathfinders to be able to address Preparing for Adulthood's top ten
messages and implement and test the proposals in the SEN and
Disability Green Paper Support and Aspiration.
The menu contains workshops that the Preparing for Adulthood
team can provide. They are not set in stone. They can be adapted to
meet local needs and will evolve as we learn more from our work
with SEND pathfinders.
